Self referral for local services

There are a number of local services that don't require you to speak with your GP for a referral. Listed below are services that you are able to make a self referral to, reducing the time before you can access the service.

Complete this form and one of our practice staff will be in contact to arrange an appointment to discuss your referral.
Waiting times NHS App

If your doctor has referred you to a specialist, you will need to book your appointment using the ‘NHS e-Referral’ system or the NHS app.

You will receive a letter within 2 weeks with details on how to book your appointment and any passwords you may need.

To track your e-Referral, you will need the following details to log into the e-Referral website:

  • Your date of birth
  • Reference number (on the first page of your letter)
  • Password (on the second page of your letter)
